The Cerecons Community Health Record (CHR) is a private cloud-based platform for clinical integration, medical management and patient engagement using Community-based clinical records and charting for Meaningful Use measurements.
An ONC-ATCB certified and industry-recognized healthcare portal solution, CERCONS is uniquely different due to its design as a Coordinated Care, Community-based System in wherein data is entered, re-used and shared automatically via HIE tools, which provides historical patient information. In addition, patient information is pre-loaded from membership eligibility and clinical history data.
While the majority of electronic health and medical records vendors have built their systems around Practice-based Data Entry Models that do not support MU-mandated criteria for data sharing and interoperability, CHR was developed with a single record, for each patient, which is shared among practitioners. This infrastructure provides the fundamental elements for quick implementation and meets core competency of MU requirements including security, privacy and interoperability.
